Johnathan will provide a free 10 minute initial consultation over the phone to assess your needs for therapy. Use the contact information below to set up a consultation call.
Fees
Initial Assessment Session (60 minutes) — $100
Individual sessions (45-60 minutes) — $80
Couples sessions (45-60 minutes) — $100
Family sessions (45-60 minutes) — $100
Medicare
As an Licensed Professional Counselor, Johnathan is not able to accept Medicare. A Licensed Professional Social Worker or a Psychologist will be able to take Medicare insurance.
Cash discount for Medicare patients who want to see Johnathan: $60 per session.
Forms of payment
All payments are accepted as either card (HSA or credit), cash or check directed to Olive Tree Counseling, LLC. Payments for sessions are collected before or at the end of each session. Insufficient fund checks will automatically be sent to a collection agency for processing.

What is the procedure for reimbursement?
Johnathan’s counseling services are reimbursable for out-of-network benefits by most insurance plans. If requested, he will gladly generate an invoice for you to file claims with your carrier. Again payment is made to him during time of service, and you are responsible for contacting your insurance company for reimbursement with invoice.
Employee Assistance Programs
Johnathan is a provider for several employers in the Randolph County area. If you contact your EAP benefits or Human Resources representative, they will assist you with providers in your area and assign you 4-8 sessions that are free to you based on your health benefits with your company/school. If follow up sessions are needed after EAP sessions are used, you can request further sessions or switch to your insurance coverage.
